29 CFR 1926.352(i): Drums containers, or hollow structures which have contained toxic or flammable substances were not, before welding, cutting, or heating was undertaken on them, either be filled with water or thoroughly cleaned of such substances and ventilated and tested: a) On or about 2/7/14 at the old American Axle Plant located at 1057 East Delevan Avenue, Buffalo, New York; employees were using cutting torches to remove and salvage equipment from seperator tanks in the waste water treatment plant. A tank that contained, or had contained, recovered cutting fluids ingnited while employees were expanding an opening to remove a mixer. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about 2/7/14 at the old American Axle Plant located at 1057 East delevan Avenue, Buffalo, New York; employees were on the second level of the waste water treatment facility salvaging equipment from the building. There were (6) 8'x8' floor holes along the west wall adjacent to the work area. There was no fall protection in place when employees passed through this area. NO ABATEMENT CERTIFICATION REQUIRED
