29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:   On or about February 18, 2014, employees were exposed to eye injuries when the employees were not required to wear eye protection when using chemicals such as, but not limited to, EPO-TEK 353ND epoxy.  Part A and B recommend the use of safety goggles.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about February 18, 2014, employees were exposed to struck-by hazards when the employer had not developed a lockout/tagout program with training and machine specific procedures.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about February 18, 2014, employees were exposed to chemical hazards when the employer had not trained the employees on the new hazard communication standard and the new Safety Data Sheet (SDS) format.  Chemicals such as, but not limited to, propane and EPO-TEK 353ND (epoxy phenol novolac) are used at the site.
