29 CFR 1926.307(d)(1): Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs  (m) and  (o) of this section:Note:  Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.    On or about February 14, 2014, on the west side of the construction site, employees utilized an air-compressor with an unguarded pulley exposing employees to caught-by hazards.

On or about February 14, 2014, on the west side of the construction site, employees engaged in roofing activities without fall protection were exposed to a fall hazard of approximately 22 feet to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about February 14, 2014, on the west side of the residential home, employees using an extension ladder to access the roof without the side rails extending 3 feet above the landing were exposed to a fall hazard of approximately 22 feet to the ground below.
