29 CFR 1926.1052(c)(5): Handrails and the top rails of stairrail systems were not capable of withstanding, with failure, a force of at least 200 pounds (890 n) applied within 2 inches (5 cm) of the top edge, in any downward or outward direction.  a)  On February 12, 2014 at the Herman Cortez Avenue, apartment construction project; where 2x4 boards used as the toprail were nailed together without adequate support.
