29 CFR 1926.403(b)(1)(vii): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: Other factors which contribute to the practical safeguarding of employees using or likely to come in contact with the equipment. a. The employer failed to inspect the extension cords to ensure that they were free of known hazards; the extension cords were run across the main road leading into the site, submerged in 2 inches of water and exposed to vehicular traffic damage. The employees on the site were exposed to a contact with electrical current hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. The employer had extension cords run from a temporary electrical source across the primary access road to the site which resulted in the cord being exposed to damage from being run over. The employees were exposed to contact with live electrical.
