On or about February 20, 2014 the employer did not provide ppe training to the maintenance employee who is provided safety glasses, face shield, nitrile gloves, and coated gloves to replace 55 gallon drums of corrosive 30-33% hydrogen peroxide at the chemical station adjacent to the sort area.

On or about April 04, 2014 the employer provided an Underwriters Laboratories listed relocatable power tap connected to a flexible cord and plug rather than directly to a receptacle to supply power to the wrapping table at the number four ironing machine in the Production Area.  b.  On or about April 04, 2014 the employer did not securely fasten in place a four inch square metal receptacle box used to supply power to the cord and plug connected wrapping table at ironer #1.
