29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    (a) Southeast, packaging hall: on February 19, 2014, the southeast corner emergency exit door access and associated egress path were blocked a pallet containing empty bottles and a bottling line in-feed tray.    (b) Southwest, packaging hall: on February 19, 2014, the southwest corner emergency exit door access and associated egress path were blocked by a bottling line conveyor.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Southwest, packaging hall employees' desk and Citronix coding workstation, orange unmarked extension cord and orange extension cord Cat#112-5 E70726: on February 19, 2014, employees were exposed to electric shock hazard where listed and labeled electrical equipment was not installed and used in accordance with the manufacturers' provisions and other applicable listing standards for that type of equipment. Energized extension cords were not rated for use in wet locations and remained in service where damaged outer jacket was wrapped with black electrical tape.    (b) Southwest, packaging hall employees' desk, Ace Hardware relocatable power tap model no. 33543 E116852 4: on February 19, 2014, employees were exposed to possible equipment overheating resulting in fire and burn hazards where listed and labeled electrical equipment was not installed and used in accordance with the manufacturer's provisions and other applicable listing standards for that type of equipment. Energized relocatable power tap was not used and installed as designed because it was utilized in the industrial setting wet location as a substitute for fixed wiring and not directly connected to permanently installed branch circuit receptacle.    (c) Southwest, packaging hall by Sigrist sensors, Belkin travel surge protector model no. BE 107201 08: on February 19, 2014, employees were exposed to fire and burn hazards from possible equipment overheating and other malfunctions where listed and labeled electrical equipment was not installed and used in accordance with the manufacturer's provisions and other applicable listing standards for that type of equipment. Energized relocatable power tap was not used and installed as designed because it was utilized in the industrial setting wet location without direct connection to permanently installed branch circuit receptacle, and connected in series to other relocatable power tap via extension cord.     There is no abatement certification or documentation required for this item.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    Northwest, fermentation tanks, multiple unmarked extension cords and flexible cords: on February 19, 2014, employees were exposed to electric shock hazard when walking in-between fermentation tanks to check temperature monitors, perform any cleaning tasks, and other assignments. Energized flexible cables and extension cords were suspended horizontally between tanks to connect to each temperature monitor placed on tanks above the adjacent floor.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
