29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,  a. On the blue trans loader where employees access the railcar tankers, the trans loader railcar access platform was not extended and positioned adjacent to the railcar, thereby exposing employees to a 12-13 foot fall hazard.   b. On the black trans loader where employees access the railcar tankers, the trans loader railcar access platform was not positioned adjacent to the railcar, thereby exposing employees to a 12-13 foot fall hazard.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998]  a. In the office trailer where employees charged lead acid batteries, an eye wash station that meets the minimum requirements of a 15 minute supply of drenching water was not available, thereby exposing employees to a contact hazard.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.  a. In the storage shed where employees stored uniforms, tools, and equipment, the circuit breaker panel dead front was not installed, thereby exposing employees to a contact hazard.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information.  a. At the worksite where employees used and transferred chemicals to include, but not limited to liquefied petroleum gases and crude oil, paints, and cleaning chemicals, employees had not been trained on the 2012 HAZCOM requirements to include labels, pictograms, signal words and the Safety Data Sheet (SDS) format.
