OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to exposed to struck-by and crush-by hazards: Location - 18 Wellesley Street, Weston, MA: On or about 1/13/14, an employee was operating a Waltco flip-away lift gate on the back of a commercial box truck and the weight of the flip-away lift gate came down to the ground, amputating the tip of the employee's left middle finger and breaking the bone of the employee's left middle finger.
