29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     a) Roof - On or about, 02/06/2014, carpenters installing cedar shakes with a pneumatic nail gun were not provided with head protection.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform was more than 14 inches:     a)  SE Corner of House - On or about, 02/06/2014, a carpenter was nailing cedar shakes onto the exterior wall of a house from the platform of a pump jack scaffold whose platform was located (horizontally) 16 in. from the exterior wall.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a)  Roof - On or about, 02/06/2014, carpenters installing cedar shakes and removing debris from a low-sloped roof were exposed to a 11 foot fall from an aluminum extension ladder which did not project 3 feet above the ladder landing or that was not secured in place and provided with a grabrail.
