29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: (a)GG Multi-Services, LLC @ 170 S Jackson Street, Denver, CO 80209: On and before February 14th, 2014, the exposing employer did not ensure that employees were working on a residential re-roof project with proper fall protection. Two employees were tearing off roof material near the edge of a multi-complex residential roof greater than 20 feet, without being tied-off. This condition exposed employees to fall hazards.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. (a) GG Multi-Services, LLC @ 170 S Jackson Street, Denver, CO 80209: On and before February 14th, 2014, the exposing employer did not ensure that employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns were protected by hard hats. An employee working on the ground was not wearing a hard hat while working under falling and flying debris from the roof. This condition exposed employees to struck-by hazards.
