29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: a. 3180 MJT Corp. dba Albert's Cafe (First Floor, Garage): The employer ran a flexible extension cord that provided electric power to a Strato-Lift TRX-13 Scissor Lift across the floor in front of the entrance door to the garage. This condition exposed employees to a tripping hazard when they walked into the garage. Observed on or about May 14, 2014

29 CFR 1910.303(b)(8)(i): Electric equipment was not firmly secured to the surface on which it is mounted: a. 3180 MJT Corp. dba Albert's Cafe (First Floor, Garage): The employer did not properly mount and secure a metal receptacle box. A long flexible extension cord that provided electric power to a Strato-Lift TRX-13 Scissor Lift was plugged into the outlet box which was not properly mounted and secured. This condition exposed employees to an electrical shock hazard. Observed on or about May 14, 2014

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a. 3180 MJT Corp. dba Albert's Cafe: (First Floor, Garage): The employer laid a flexible extension cord that provided electric power to a Strato-Lift TRX-13 Scissor Lift along of the floor, directly in front of the doorway, which was not protected from damage. Employees who entered the garage from the outside of the building could step on the cord. This condition exposed employees to an electrical shock hazard. Observed on or about May 14, 2014
