29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about 02/20/2014, on the work site located at 4243 Ambassador Caffery Parkway, Lafayette, LA 70508, the employer did not ensure that the Skilsaw (Work Drive Saw - #SHD77M) utilized to make cuts to set form had a permanent and continuous path to ground.  The plug was missing the ground pin.
