29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) On or about February 20, 2014, and at times prior thereto, employee was using a pneumatic gun to install siding material and was exposed to eye hazards. Eye protection was not being worn to prevent injury. Note: Abatement certification is required for this item.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph. (a) On or about February 20, 2014, and at times prior thereto, two employees were installing siding from an aluminum scaffolding without using safety netting between the two levels of aluminum planking and were exposed to fall hazards of approximately seventeen (17) feet on both the sides and ends of the scaffolding. Note: Abatement certification is required for this item.
