29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   6251 McPherson Avenue, St. Louis, Second Floor: Employees were exposed to fall hazards where a hole in the floor which measured approximately 5 feet long and 13 inches wide with a fall distance of approximately 12 feet to a lower level, was not protected by a cover or guardrail systems.  The exposed employees were not provided personal fall arrest systems and were not required to use personal fall arrest systems.
