29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    a) Circuit Breaker Panel on the First Floor of Medical Office Building: On or about February 14, 2014, employees worked bare-handed on an energized panel rated at 800 Amps and 480 Volts while adding fingers to install circuit breakers.  The employer had not trained employees in the recognition and avoidance of hazards while working on energized panels or to de-energize such panels prior to the commencement of work.    Abatement certification and documentation are required for this item.

29 CFR 1926.416(a)(1): The employer permitted employees to work in such proximity to any part of an electric power circuit so that the employee could contact an electric power circuit in the course of work, and employees were not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means.   a) Circuit Breaker Panel on the First Floor of Medical Office Building: On or about February 14, 2014, employees worked bare-handed on an energized panel rated at 800 Amps and 480 Volts while adding fingers to install circuit breakers.  Employees performed work on an energized panel as opposed to de-energizing the circuit and grounding it, or by guarding the energized circuit by insulation.   Abatement certification and documentation are required for this item.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition when it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a) Circuit Breaker Panel on the First Floor of Medical Office Building: On or about February 14, 2014, employees worked bare-handed without any personal protective equipment on an energized panel rated at 800 Amps and 480 Volts while adding fingers to install circuit breakers.      NOTE: National Fire Proteciton Association NFPA 70E-2009, Table 130.7(C)(9) defines work on panelboards or switchboards rated >240 Volts and up to 600 Volts as hazard/risk category 2.  Personal protective equipment required by this category includes, but is not limited to, flame resistant (FR) clothing with a minimum arc rating of 8, a hard hat, safety glasses or safety goggles, hearing protection, leather gloves and leather work shoes.    Abatement certification and documentation are required for this item.
