29 CFR 1926.451(a)(1): Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.   a)  14 Maplewood Ave., Rumson, NJ:  On or about 2/8/2014, a 5 Stars Builder LLC employee was working on a job-built bracket scaffold made from 2x4 lumber.  The scaffold was not capable of supporting the worker and collapsed.  The worker fell over 14 feet to the ground sustaining serious injury.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked.    a)  14 Maplewood Ave., Rumson, NJ:  On or about 2/8/2014, a 5 Stars Builder LLC employee was working on a job-built bracket scaffold made from 2x4 lumber.  The walking working surface consisted of a single 2x8 piece of lumber.  The scaffold deck was not fully planked.  The worker fell over 14 feet to the ground sustaining serious injury.

The walking working surface consisted of a single 2x8 piece of lumber.  The scaffold platform was not at least 18 inches wide.  The worker fell over 14 feet to the ground sustaining serious injury.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:   a)  14 Maplewood Ave., Rumson, NJ:  On or about 2/8/2014, a 5 Stars Builder LLC employee was working on a job-built bracket scaffold made from 2x4 lumber.  The scaffold had not been erected under the direction of a competent person qualified in scaffold erection.  As such the scaffold was not capable of supporting the worker and collapsed.  The worker fell over 14 feet to the ground sustaining serious injury.

The employee was not protected from the fall hazard by guard rails or PFAS.  The worker fell to the ground, sustaining serious injury.
