29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): Duce Construction Company had an employees exposed to cave-in hazards at depths ranging from 5 ft., 10 in. to approximately 10 ft. and the employer did not provide cave-in protection in accordance with 1926.652(a)(1), on a construction site in Savoy, IL. These cave-in hazards included: a. East end of excavation at depth of 5 ft., 10 in., with walls of trench not adequately benched in accordance with soil classification and b. Work from within manhole box at depths of approximately 10 ft., with walls of excavation not consistently sloped back in accordance with identified soil type from a minimum of 18 in. below top of shield to ground level.
