29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. At the second floor, employees were exposed to a fall hazard 11 feet 4 inches, in that, guardrails were not installed at the stairway opening where an extension ladder was placed to access the second floor.   b. At the second floor, employees were exposed to a fall hazard 11 feet 4 inches, in that, guardrails were not installed at the north and south material handling areas.   c. At the second floor, employees were exposed to a fall hazard 11 feet 4 inches, in that, guardrails were not installed at the elevator shaft doors.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:   a. At the second floor of the building, employees were exposed to a 12 feet fall hazard to the ground below, in that, the window openings which were 8 inches from the walking surface were not provided with standard railings.

At the second floor, employees were exposed to the hazard of impalement, in that, protruding rebar was not capped throughout the entire second story where masonry wall construction was in progress and at material handling areas.
