29 CFR 1910.252(b)(2)(iii): Workers or other persons adjacent to the welding areas were not protected from the rays by noncombustible or flameproof screens or shields or were not required to wear appropriate goggles:  a) Arc Welding Workstations: On or about February 18, 2014 and at times prior thereto, the employer did not ensure workers and/or other persons in the adjacent areas were required to use eye protection (e.g. welding curtains, tinted safety glasses, and/or an equivalent) which exposed employees to eye injuries associated with exposures to welding rays.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either the information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers or with the product identifier and pictures, symbols or combination thereof which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available  to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a) Production Area: On or about February 18, 2014 and at times prior thereto, the employer did not ensure a secondary container was labeled which exposed employees to chemical hazards associated with Hold Tight 102.
