29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.     On or about 2/21/2014 employees at the Stork Club snow removal project were exposed to approximately 16' falls.  Employees were engaged in snow removal without fall protection.
