29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Location: 133 eastern Avenue, Lynn MA 01902 On or about 2/20/2014, employees were not protected from falling while working on a roof 5 stories above a lower level, and on stairwell landings, where there was exposure to rough wall openings less than 39 inches in height from the floor level. The (Sapphire Construction, Inc.) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 762841, citation number 01, item number 002 and was affirmed as a final order on (3/20/2013), with respect to a workplace located at 96 Park Avenue, Tewksbury, MA 01876.
