29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On or about February 18, 2014:  a) Employees were exposed to electrical shock and or burns when the employer installed nonmetallic sheathing cable for temporary power to use within the structure being constructed. The cable being used was laid upon the ground and was not protected.

29 CFR 1926.405(a)(2)(ii)(A): Temporary wiring feeder conductors were not run as multiconductor cord or cable assemblies or within raceways; or, where not subject to physical damage, run as open conductors on insulators not more than 10 feet apart:  On or about February 18, 2014:  a) Employees were exposed to electrical shock and burns when the employer failed to protect nonmetallic sheathed cable that was being used for temporary power on the work site.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about February 18, 2014:  a) Employers were exposed to electrical shock or burns when the employer failed properly guard the non-metallic sheathed cable that had been spliced and was covered by electrical tape as a form of insulation.
