29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 289 Wells St., Greenfield MA On 2/21/14 and at times prior, the rotating shaft ends of numerous chip-breaking, slab-and-gashing-milling, and two-slot-milling machines were not guarded, exposing operators and others in the vacinity to caught by hazards.
