29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  (a) Allegro Coffee Company  at 12799 Claude Court, Thornton, CO 80241: On and before 2/21/14, the employer did not ensure the compressed air nozzles used in the production area near burner R1000 was reduced to less than 30psi.  This condition exposed employees using compressed air hoses to the hazard of an embolism in the blood stream.   Allegro Coffee Company, was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1910.242(b) which was contained in OSHA Inspection Number 313413619, Citation Number 1, Item 1 issued on 12/15/2009, and became Final Order on January 8, 2010, with respect to a workplace located at 12799 Claude Court, Thornton, CO 80241.

29 CFR 1910.134(c)(2): The employer did not develop and implement a respiratory protection program in accordance with sections i and ii of this paragraph (c)(2) for employees who chose to wear respirators where respirator use was not required:  (a) Allegro Coffee Company  at 12799 Claude Court, Thornton, CO 80241: On and before 2/21/14, the employer did not establish and implement a written respiratory protection program necessary to ensure that employees voluntarily using a full face Survivair respirator affixed with Sperian 100 organic vapor cartridges are medically able to use the respirator and that the respirator is cleaned, stored and maintained so that its use does not present a health hazard to the user. This condition exposes employees to respiratory hazards associated with the use of negative pressure respirators.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Allegro Coffee Company  at 12799 Claude Court, Thornton, CO 80241: On and before 2/21/14, the employer did not ensure that any employee using a respirator voluntarily was medically able to use the respirator.  The employer provided tight-fitting facepiece respirators for voluntary use and did not ensure that employees received a medical evaluation prior to initial use of the respirator.  This condition may exacerbate an underlying medical condition.

29 CFR 1910.134(k)(3): Training was not provided prior to requiring employees to use a respirator in the workplace:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD)  (a) Allegro Coffee Company  at 12799 Claude Court, Thornton, CO 80241: On and before 2/21/14, the employer did not provide comprehensive, understandable annual respirator training prior to employee use of respirators in the workplace.  Employees use full face survivair respirators with organic vapors cartridge and N95 #2350 respirators when working in environments that are dusty from cleaning operations or coffee bean transfer from burlap bags.  This condition exposes employees to a respiratory hazard due to improper fit, usage, and maintenance.  Abatement Note:  The employer shall ensure that each employee can demonstrate knowledge of at least the following: (1)  Why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect of the respirator;  (2)  What the limitations and capabilities of the respirator are; (3)  How to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions; (4)  How to inspect, put on and remove, use, and check the seals for the respirator; (5)  What the procedures are for maintenance and storage of the respirator; (6)  How to recognize medical signs and symptoms that may limit or prevent the effective use of respirators; and, (7)  The general requirements of this section.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a)   Allegro Coffee Company  at 12799 Claude Court, Thornton, CO 80241: On and before 2/21/14, the employer did not ensure that any employee using a respirator voluntarily was provided with the information contained in Appendix D to this section when the employer determined that any voluntary respirator use was permissible.  The employer provided full face Survivair respirator affixed with Sperian 100 organic vapor cartridges to employees who clean out the "green coffee dump station" located in the production area.  This condition exposed the employees to a respiratory hazard.
