29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  A)     At the Abernathy Fish technology center, did not include all confined spaces at the facility in their workplace evaluation.  An example of confined spaces not evaluated included, but were not limited to the large tanks in the water tower used to add oxygen to the facility water supply.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  A)    In the extruder room, in the nutrition section, as well as other places throughout the facility, the employer did not have lockout procedures developed to make sure all sources of energy were isolated from the equipment being maintained.  An example of equipment that needed lockout procedures included, but was not limited to the large fixed extruder used to make fish food, in the nutrition section of the facility.

29 CFR 1910.25(d)(1)(x): Ladders were not inspected frequently and those which have developed defects were not withdrawn from service for repair or destruction and tagged or marked as "Dangerous, Do Not Use.":  A)  In the Nutrition section of the facility underneath the food extruder, there was a damaged 4 foot high wooden step ladder that was showing signs of damage, that was still being used by employees.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  A)     In the generator room, and adjacent storage room, fluorescent lights were 7 feet 9 inches from the floor and were not protected from damage.
