29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: On or about February 28, 2014, and at times prior thereto, an employee was operating the Verson model 1062 and Atek's Bantam model B4120MFO press brakes with no point of operation guarding. Employee was exposed to crushing or amputation hazards.
