On or about February 22, 2014, at the 48 Shuttle Meadow Avenue, New Britain jobsite, east side of the roof: Three employees were on an 8-pitch steep roof, thirty feet from the ground below, shoveling snow and removing shingles, and were not protected from falling by any means of fall protection.

On or about February 22, 2014, at the 48 Shuttle Meadow Avenue, New Britain, west side, rear of the jobsite: An employee, carrying a bundle of materials in his right hand, was observed climbing an extension ladder to the roof level of the house. The employee was unable to fully grasp the rungs or side member of the aluminum ladder, maintaining three points of contact at all times.
