29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:    a)  #1267 Goff Brook Plaza - On or about 2-20-14 - Employees were observed using flexible extension cord to power a circular saw and chop saw that was run in wet conditions and not equipped with a ground fault circuit interrupter in the system, exposing employees to electrical shock hazard.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:    a)  #1267 Goff Brook Plaza - On or about 2-20-14 - Employees were exposed to electrical shock hazards from flexible extension cord run under metal doorway.  The lead cord was not connected to a ground fault circut interrupter and in wet conditions.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a)  #1267 Goff Brook Plaza - On or about 2-20-14 - Employee was using a Hitachi, 7-1/4 inch circular saw, Model C75B2, that had the strain relief for the flexible connection cord broken at the handle exposing wiring to the switch.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a)  #1267 Goff Brook Plaza - On or about 2-20-14 - Employee was observed installing drywall/insulation while standing on a platform 36 inches wide and 12 feet above the ground with no fall protection being used.       The Generation Dry Wall, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.510(b)(1), which was contained in OSHA inspection number 913325, citation number 1, item number 1 and was affirmed as a final order on 11/4/13, with respect to a workplace located at Wal-Mart, 495 Flatbush Avenue, Hartford, CT.
