29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section:      (a)       DAG Custom Homes, 2601 Belford Ave., Grand Junction, CO 81501:  On and before January 28, 2014, as the controlling contractor onsite, DAG Custom Homes did not ensure that the supported scaffold was designed by a qualified person, in that the scaffold wood platform was constructed at the job site by non qualified personnel. A skid steer was used to elevate an employee approximately 15 feet on a job made scaffold in order to perform siding work. This condition exposed the employee to a fall hazard of approximately 15 feet.      Abatement Note:  Abatement certification is required for this item (see enclosed Abatement-Certification Letter").
