29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas: On or about February 19, 2014, and times prior to, at the worksite located at 311 Arkansas Avenue in Fayetteville, Arkansas, an excessive accumulation of wooden boards, some with protruding nails, were scattered on the ground at the west end of the sleep wing. This condition exposed employees to falls and punctures.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: On or about February 19, 2014, and times prior to, at the worksite located at 311 Arkansas Avenue in Fayetteville, Arkansas, a midrail was not provided on the west end of the cable guardrail system surrounding the perimeter of the 3rd floor sleep wing roof deck level. This condition exposed employees engaged in setting forms to unprotected falls of greater than 6 feet.

This condition exposed employees engaged in setting forms to unprotected falls of greater than 6 feet.
