J & M Roofing Services, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315480343, Serious citation number 01, item number 003 and was affirmed as a final order on November 18, 2011, with respect to a workplace located at 10904 Lydia Estates Drive East Jacksonville FL. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. On or about February 22, 2014, on the 5:12 pitch roof, employees installing ceramic roofing tiles were exposed to an 8 feet fall hazard from the eaves in that they were not protected by a fall protection system.
