29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 22, 2014, the employer did not assure that employees were protected from ingoing nip point hazards while obtaining a speed measurement of rotating shafts at the Cameron Type 640, Winder #4 machine. A maintenance employee who used a tachometer equipped with a speed wheel to obtain speed measurements was injured when caught in the machine's unguarded nip point.
