On or about February 26, 2014, and times prior thereto, LP Branden Winches not approved for the hoisting of personnel were used to hoist employees performing maintenance.

29 CFR 1910.252(a)(2)(iv): The individual responsible for authorizing cutting or welding operations did not designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit. On or about February 26, 2014, Nabors Drilling USA, LP did not provide a hot work permit to employees of Inspection Oilfield Services prior to hot work being performed exposing employees to fire and explosion hazards.
