29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   Throughout facility - On or about February 24, 2014, employees (full-time / temporary) were required to operate / maintain / repair equipment without proper lockout / tagout procedures. the employer did not establish an energy control program for machine operators and/or maintenance employees  performing motor changes on conveyor systems and other operating equipment, thereby exposing employees to the hazard of unexpected energization of equipment.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    Throughout facility - On or about February 24, 2014, employees (full-time / temporary) were required to operate powered industrial vehicles in the performance of their duties. Operators were not trained in the safe operation of powered industrial trucks, thereby exposing all employees (full-time / part-time / temporary) in the facility to struck by hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  Throughout facility - On or about February 24, 2014, employees (full-time / temporary) were required to operate powered industrial vehicles in the performance of their duties. The employer did not perform an examination of the Crown model RC3020-30, serial number 1A189517 / Toyota model 5FBE20, Serial number 05972, to check for defects prior to / or after each shift, thereby exposing employees to struck by hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.   Production Area - On or about February 24, 2014, employees (full-time / temporary) were required to operate the Signature model 90CS, serial number FG2483 and the Gladiator Window Box Machine on a daily basis. The rotating parts of the machines were not properly guarded, thereby by exposing employees to the hazards associated with rotating parts.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.   Production area - On or about February 24, 2014, employees (full-time / temporary) are required to operate the Econo-Cut on a regular basis. The pulleys on the machine were not completely guarded, thereby exposing employees to the hazards associated with moving parts.
