29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    a) At the jobsite: inside the chicken house - Employees were exposed to fall hazards of approximately eight feet when engaged in cleaning operations near a ladderway floor opening not guarded by a standard railing. An employee fell to his death through the ladderway floor opening measuring 28 inches in width by 29 inches in length. Noted on or about 02/21/14.

29 CFR 1910.23(b)(1)(i): Every wall opening from which there is a drop of more than 4 feet was not guarded by one of the following: rail, roller, picket fence, half door, or equivalent barrier.      a) At the jobsite: inside the chicken house, at front of building, second level - Employees were exposed to fall hazards when working adjacent to an unprotected wall opening approximately eight feet above ground level. The employees were not protected from falling by a rail, roller, picket fence, half door, or equivalent barrier when bringing water hoses from outside of the building at ground level to the second level inside the chicken house. Observed on or about 02/24/14.
