(a)  At the Summer Homes Residence construction project, the stairway in building number four did not have a guard rail between the first and third floors and employees were exposed to a fall hazard of approximately 12 feet.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926.  a)  At the Summer Homes Residence construction project, building number four, guardrails were not installed on the landings between the first and third floors and employees were exposed to a fall hazard of approximately 12 feet.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  (a)  At the Summer Homes Residence Project, Guam, including but not limited to the third floor, employees were exposed to puncture and laceration hazards from lumber debris in the walkway with nails sticking out.
