29 CFR 1910.147(c)(4)(ii)(B): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:   a.  In building C, fluorescent group 26, the employer's written lockout/tagout procedure for group 26 did not include clear and specific procedures for shutting down and isolating the electric, pneumatic, and natural gas sources of energy for the D-330 Baser (North).    b.  In building C, fluorescent group 26, the employer's written lockout/tagout procedure for group 26 did not include clear and specific procedures for shutting down and isolating the electric, pneumatic, and natural gas sources of energy for the D-330 Baser (South).   c.  In building C, fluorescent group 26, the employer's written lockout/tagout procedure for group 26 did not include clear and specific procedures for shutting down and isolating the electric and pneumatic sources of energy for the K-295 Seasoner.   d.  In building C, fluorescent group 26, the employer's written lockout/tagout procedure for group 26 did not include clear and specific procedures for shutting down and isolating the electric and pneumatic sources of energy for the transfer robot (Pick and Place).   e.  In building C, fluorescent group 26, the employer's written lockout/tagout procedure for group 26 did not include clear and specific procedures for shutting down and isolating the electric, pneumatic, natural gas, and oxygen gas sources of energy for the sealers.   f.  In building C, fluorescent group 26, the employer's written lockout/tagout procedure for group 26 did not include clear and specific procedures for shutting down and isolating the electric, pneumatic, natural gas, and sulfur dioxide gas sources of energy for the Lehr.

29 CFR 1910.147(d)(2):  Machine or equipment shutdown.  The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment.  An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage:   a.  On or about January 15, 2014, in building C, fluorescent group 26, two authorized employees while performing maintenance such as evaluating and making adjustments with the guard removed on the base threader of the D-330 Baser (North) while it was in operation did not perform lockout/tagout including shutdown of the machine.  This resulted in the amputation of a portion of an employee's left index finger.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a.  On or about January 15, 2014, in building C, fluorescent group 26, two authorized employees while performing maintenance such as evaluating and making adjustments with the guard removed on the base threader of the D-330 Baser (North) while it was in operation did not perform lockout/tagout including the application of locks/tags.  This resulted in the amputation of a portion of an employee's left index finger.

29 CFR 1910.212(a)(1):  Types of guarding.  One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:   a.  In building C, fluorescent group 26, the base threader on the North side of the D-330 Baser (North) was not completely guarded on the front and each side.    b.  In building C, fluorescent group 26, the base threader on the South side of the D-330 Baser (North) was not guarded on each side.    c.  In building C, fluorescent group 26, the transfer robot (Pick and Place) that transfers lamps from conveyor A to the D-330 Baser (North) was not guarded.

29 CFR 1910.219(f)(3):  Sprockets and chains.  All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform.  Where the drive extends over other machine or working areas, protection against falling shall be provided.  This subparagraph does not apply to manually operated sprockets:   a.  In building C, fluorescent group 26, the chain and sprocket at the reload area on the North side of the conveyor to the K-295 Seasoner (North) was not guarded.     b.  In building C, fluorescent group 26, the chain and sprocket at the reload area on the South side of the conveyor to the K-295 Seasoner (North) was not guarded.

29 CFR 1910.24(h):  "Railings and handrails."  Standard railings shall be provided on the open sides of all exposed stairways and stair platforms.  Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23:   a.  In building C between fluorescent groups 26 and 24, the stairways allowing access over the conveyor from group 26 did not have intermediate rails on all of the open sides.
