29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a) East side, new residential construction located at 838 Nottingham Drive, Smyrna, GA 30082. Employee was exposed to falls of approximately 19-feet, 4-inches to the lower level while installing Hardie-Plank Lap Siding from a pump jack scaffold platform without a fall protection system in place.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    a) East side, new residential construction located at 838 Nottingham Drive, Smyrna, GA 30082: Employee was exposed to fall hazards while working from a pump jack scaffold platform at heights of approximately 19-feet, 4-inches without the supporting poles secured to the structure or provided with a means of securing the bottom of the support pole to prevent accidental movement or collapse of the system during use.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a)  Side Roof, new residential construction located at 838 Nottingham Drive, Smyrna, GA 30082: Employee installing Hardie-Plank Lap Siding from a 4/12 pitched roof at heights of approximately 21-feet, 1-inch from the lower level was not provided with a fall protection system.    (b) Rear roof, new residential construction located at 838 Nottingham Drive, Smyrna, GA 30082: Employee installing Hardie-Plank Lap Siding from a 8/12 pitched roof at heights of approximately 19-feet from the lower level was not provided with a fall protection system.
