This violation occurred on or about February 20, 2014 when the employer modified the approved installation of a GME Trench Shield (Model MH10DW NKE) using timber at the spreader ends placing a surcharge from earth and timber directly on the spreaders.  The use of timber in that fashion was not approved by the manufacturer and did not have the approval of a registered professional engineer.
