This violation occurred on or about February 21, 2014, when employees were allowed to work in an aerial lift, one (1) foot above protruding reinforcing steel without any protection from the hazard of impalement. The employee wore fall protection including a six (6) foot lanyard when the impalement hazard existed one (1) foot below the platform.
