29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Steve Swert, Perrysburg, Ohio:  On or about February 19, 2014, at the location of Lot 95, 14047 Eckel Junction, Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while removing snow from a rooftop in preparation for shingle delivery.  An employee was working on a sloped portion of the roof and was not protected from an approximate 11.5 ft. fall hazard to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Steve Swert, Perrysburg, Ohio:  On or about February 19, 2014, at the location of Lot 95, 14047 Eckel Junction, Perrysburg, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems.  Employees were exposed to falls of approximately 11.5 feet to the ground below while removing snow from a rooftop.
