29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encoutered: a) On February 26, 2014, at 4021 Hauck Rd., Sharonville, Ohio 45241, an employee was performing exhaust ventilation cleaning and associated activities on commercial restaurant roof structure adjacent to unprotected roof sides or edges without personal protective fall arrest/restraint epuipment or protected by other conventional fall protection means and was exposed to a fall hazard of 17 ft.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. (a) On February 26, 2014, the employer failed to verify that the required workplace hazard assessment had been performed through a written certification that identifes the workplace hazards. Employees were exposed to 3rd degree burn hazards from being required to utilize hot water with temps over 200 degrees. Employees were exposed skin iritation hazard(s) from being required to utilize caustic chemicals - ph. measuments over 10.

29 CFR 1910.26(c)(2)(vi)(d): If ladders are exposed to oil and grease, equipment should be cleaned of oil, grease, or slippery materials. This can easily be done with a solvent or steam cleaning. a) On February 24, 2014, in the warehouse, a metal ladder was observed with a grease build-up on some rungs.
