29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:    a.  2410 Cain Circle, Lot 15A, Puckett Manor Subdivision, Dacula, GA (Jobsite) - Employees were exposed to fall hazard.  Employees were using portable ladders that did not extend 3 feet above the upper landing while accessing the ladder to get to the roof of the residential home, on or about 02/25/14.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a.  2410 Cain Circle, Lot 15A, Puckett Manor Subdivision, Dacula, GA (Jobsite) - Employees were exposed to fall hazard.  The employees were not utilizing a means of fall protection while working and walking approximately 18-20 feet above ground level on the elevated roof, installing shingles along the ridge of the roof, on or about 02/25/14.     Francisco Javier Zetina was previously cited under Javier Cruz for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 616239,  citation number 1, item number 1 and was affirmed as a final order on October 31, 2012, with respect to a workplace located at 5480 Vista Brook Drive, Suwanee, Georgia.
