29 CFR 1910.147(c)(4)(ii): Energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 02/25/2014, in Buildings A, B, and C, the employer did not ensure that lock-out/tag-out procedures were specific to each machine (or model of machine) and included a specific statement of the intended use of the procedure, specific procedural steps for shutting down, isolating, blocking, and securing machines, specific procedural steps for the placement, removal, and transfer of lock-out/tag-out devices and the responsibility for such devices, and specific procedural steps for testing a machine to determine and verify the effectiveness of such devices and other energy control measures. Among other methods, one feasible and acceptable means of abatement includes, but is not limited to, ensuring that a clear and specific lock-out/tag-out procedure is developed and utilized for each machine or type/model of machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure(s) at least annually to ensure that the procedure(s) and the requirements of this standard were being followed: On or about 02/25/2014, in Buildings A, B, and C, the employer failed to conduct an annual inspection of each lock-out/tag-out procedure. Among other methods, one feasible and acceptable means of abatement includes, but is not limited to, ensuring that a periodic inspection of the lock-out/tag-out procedures are conducted at least annually, performed by an authorized employee other than the one utilizing the procedure being inspected, conducted to correct any deviations or inadequacies identified, and include a review of each authorized employee's responsibilities under the procedure being inspected.
