(c)  On the roof north of the balcony and above the entrance to central unit of the triplex at 1414 Lone Shore Dr.:  on February 25, 2014 and at times prior thereto, employees engaged in exterior construction activities were exposed to  falls from approximately 8 feet onto concrete while transiting the roof to access the roof.    (d)  Second floor, central unit of the triplex at 1414 Lone Shore Dr.:  On February 25, 2014 and at times prior thereto, employees engaged interior and exterior construction activities were exposed to falls from approximately 10 feet onto concrete while working near the inadequately guarded stairwell.
