29 CFR 1926.950(c)(1)(i): An employee was permitted to approach closer to exposed energized parts closer than shown on Table V-1 and the employee was not insulated or guarded from the energized part (gloves or gloves with sleeves rated for the voltage involved shall be considered insulation of the employee from the energized part), or An employee was exposed to electrocution hazards while installing a new utility pole and the employer did not ensure that the employee was properly insulated from the energized part by the use of gloves or gloves with sleeves while approaching the energized part.
