29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: A. On or about 2/7/2014 at 2800 Browns Bridge Road, Gainesville, GA; employees performing charging operations were exposed to electrical burns and shock where the extension cord that was plugged into the temp power pole was missing its grounding pin. B. On or about 2/7/2014 at 2800 Browns Bridge Road, Gainesville, GA; employees performing charging operations were exposed to electrical burns and shock where the power strip that was plugged into an extension cord that was powered by the extension cord in item A, was missing the grounding pin.
