29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.   a)       Building 8, Diving Center;  A guardrail system on the blue metal platform on the second floor was missing a midrail, exposing employees to potential fall hazards.   The National Oceanic and Atmospheric Administration was previously cited for a violation of this standard which was contained in OSHA inspection no.  906829 citation 2, item 1, issued on June 4, 2013 with respect to a workplace at Pasco, Washington . 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   (a)  Building 8, Diving Break Room;  Two white relocatable power taps were daisy chained and mounted in a cabinet, violating the UL listing and labeling. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  The UL listing also stated that relocatable power taps are not to be mounted. This exposes employees to possible electrical fire hazards.    (b) Building 8, Diving Boat Bay;  A Maytag washing machine was plugged into an extension cord.  The UL listing states that extension cords are to be used on a temporary basis only. This exposed employees to possible electrical fire hazards.

29 CFR 1910.305(j)(1)(i): Rosettes and cleat-type lampholders and receptacles located less than 2.44 m (8 feet) above the floor had exposed terminals:   a)  Building 8, Diving Center, Exercise Room;  A florescent light bulb was located 7 feet 2 inches from the floor and was not guarded.
