29 CFR 1910.106(f)(6):  "No Smoking" signs were not conspicuously posted where hazard from flammable liquid vapors is normally present:    In the lumberyard, east of the lumber treatment building.  On or about February 26, 2014, and at times prior thereto, employees were exposed to an explosion, as there were no "No Smoking" or "Flammable" warning signs on or near the 2,000-gallon container of diesel fuel.

29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    Throughout the lumberyard, on or about February 26, 2014, and at times prior thereto, employee(s) were exposed to crushing injuries from vehicle rollover, while transporting stacks of lumber and plywood around the lumberyard in powered industrial vehicles without safety belts installed.

29 CFR 1910.178(l)(6):  The employer did not certify that each operator of a powered industrial vehicle had been trained and evaluated as required by this paragraph:     On or about February 26, 2014, and at times prior thereto, employee(s) were exposed to injuries from vehicular accidents while operating a Mitsubishi Lift Truck, Model FD35, Type D, Serial no. T12A-02054, at the worksite, without seatbelts and without proper certification to ensure the training and awareness of surface conditions, maneuverability, carrying capacity, and the use of safety belts while operating the vehicle.
